With the Patient Portal, you can:
- Schedule your own appointment - from home or work
- Email to ask non-urgent questions - about billing, your visit or resources
- Request a prescription refill - from home or if you're on vacation and forget your pills
- Input your personal information and your medical history - online and secure
- Access your chart 24/7 - might come in handy if you are in the emergency room somewhere
- Check your "to do" list of health maintenance items or tests; look for the handouts you lost when you left the office
- Pay your bills online
How to get access and log on:
1. Provide your email address, which will be your username
2. Contact Sustaina Center for Women for a temporary password
3. Go to sustainacenter.com
4. Click on the Login button in the orange Patient Portal section
5. Enter your email address and temporary password, you will be asked to provide a new password upon login
Before you come in for your first visit, or to update your information at any time, click on the patient information icon to input your personal and medical information to save time in the office.
